Hello! Since the shadowing is present in different lighting it could be that there is more volume above the treatment area, hence causing a shadow when you look at the area with overhead lighting. It is hard to tell from the pictures exactly what the concern is.
We suggest going in for a follow up consultation, if possible with the injector who performed your initial treatment.

There are a few factors that play into potential swelling, including the area (lips swell more than other areas), and technique used by your injector (needle vs cannula)
We typically experience minimal to moderate swelling with Juvederm Ultra. I have worked with Emervel Lips in the past and found the swelling to be about the same, when done in the same technique and area, as compared to Juvederm Ultra.

Number one, ensure you are on a good pharmaceutical grade skincare line, and are using antioxidants, SPF and chemical exfoliants (such as Retinol) to prevent any fine lines from getting worse and to help with the overall skin quality. Hello! I would agree with the recommendation by your injector. Building structure in the cheeks is important to support the tear trough area. Often times working with cheek fillers the tear troughs will improve even without adding filler to the tear trough directly.
In our clinic, we almost always add cheek filler prior to doing tear troughs.
