You have tuberous breast, but you also have ptosis. You have good volume. I would recommend a mastopexy, inverted T or anchor type.

Unfortunately some patients get scarring. Sounds like you have done a lot of the basic treatments that can be done. I would recommend laser treatments. I would do one treatment every 3 weeks. I would recommend fraxel laser. If you do not get improvement you could consider scar revision.

I would agree with one of the other surgeons that one cannot completely put such a large implant at 750 under the muscle. About 50 percent of the implant will be under the breast.
If you want to be that large, then you would need to have the implant placed under the breast. You may get some stretch marks. I would also recommend a cohesive SCF type implant for Allergan. They work very well under the breast, and have very low wrinkling. You would not need a mini lift in my opinion, if you go...

I will be very honest, you have large breasts. It is very difficult to make your chest look masculine, and try and maintain nipple sensation. It takes some time for the nipple to declare and to know if you have lost the nipple. I do free nipple graft techniques, In this type of surgery the nipples areola complex is removed and replaced as a graft. It takes a few days to see if the graft is taking. In a technique where the nipple complex is retained in the surgery, on a pedicle of tissue, one can usually see a discoloration in the nipple. Please consult with your surgeon and discuss your concerns.

Generally there are 4 incisions approaches to breast augmentation. And you have mentioned them all. In younger women, who have not had children I prefer infra mammary. Many times the Areola size is too small to do a peri areolar approach. Also it's a straight approach to the augmentation and minimizes the incisions into the breast itself. This is very helpful for breast feeding down the road. In women who have larger areola I like peri areolar. It heals nice and looks great....
