This upper flap can not be treated with aggressive liposuction at the time of surgery because it is possible to compromise its blood supply and negatively effect healing. I would have liked to see a pre-operative picture of your breasts, however since this is unavailable I will comment without it. If the breast asymmetry is only present on muscle contracture and the breasts appear normal otherwise, I would not advise additional surgery. If the asymmetry persist with relaxed muscles, one might consider further release of the lower pole of the left breast. Breast distortion with pectoralis major contraction is normal.

The best treatment for this problem is surgical removal of the blood in the pocket. Sometimes these type of blood collections can resolve without surgical intervention, but you will have a higher chance of complications including hardening of the breast in the future.

I agree with the other surgeons who have reviewed this question. The story is confusing. It is noteworthy that a size reduction of 120 ccs is not excessive. This probably represents about a single cup size change in your situation. It is imperative to obtain your operative records and all your implant information. If the implant information is not available from your surgeon's office, you will surely be able to obtain it from the facility where the procedure was performed.
