Looking at your photo, I have to wonder if your areolae were very protrusive and stretched out pre-operatively. In some patients, when this is the case pre-op, after the tissue is removed and the areolae shrink down, the skin doesn’t always tighten fully, and you see creases and a lax appearance to the skin. Most of the time, this improves over the first year post-operatively, but if it doesn’t, sometimes peri-areolar tightening can help to improve the appearance.

From your photos, it appears that you may have been over-resected on the left with a resultant crater deformity. If it’s only 11 days out, there is swelling that will dissipate such that the depression may not be as bad with time. If the appearance persists past the six to eight week mark, you might need to have a revision to address it. You should discuss this with your surgeon.

After massive weight loss of the sort you’ve had, it can be difficult to get an adequate amount of skin out, even when the vertical resection is done as well as removing the skin at the bottom. It’s difficult to comment on your result without having seen the pre-operative photos as well, but it appears that additional skin removal would be required to give you a better contour. Seromas occur on occasion, often following some bleeding post-operatively, and they are easily managed with serial aspiration. If you had 2 lbs. of tissue from each side, I’m wondering if you had skin resection with drains. If the drains were removed early or if no drains were used, the seroma may have resulted from that. By now, I’m guessing this was resolved, but if not, a drain would be useful to keep it compressed down and empty.

At 7 weeks after surgery, there is still swelling that needs time to resolve, and often the remnant that’s left under the areola will swell and be more palpable in the early post-op period. It really takes months for a final result, and I advise patients that swelling is present for up to six months, though it might be very subtle. Have patience.

As the other surgeons have noted, at this early point, you have significant swelling. If you’re wearing compression, that swelling tends to be controlled a little better, and when you discontinue the compression, you often have the impression that the results aren’t as good as the tissue returns to its normal non-compressed state. Thanks for your question. Your photos and description show mild gynecomastia, and at least in Massachusetts, it would not be covered by insurance.
