Your profile view shows a moderate amount of abdominal distention, but only through exam can your plastic surgeon determine if that is entirely fat or separation of your abdominal muscles. If it is primarily fat and you have good skin tone then liposuction would be appropriate. Otherwise and tummy tuck would be your best choice.

You state that your pannus has been previously removed, but you are unhappy with your "huge stomach". Your pictures do not reveal your current situation, but it sounds like a combination of fat and separation of your abdominal muscles (diastasis recti). Liposuction can reduce the fat volume, but if you are after a 'smaller' abdomen, you will probably require tightening of the abdominal muscles, which is done with a tummy tuck.

You are going for a fairly large size implant. If it is going to be placed beneath your pectoralis muscle I believe you will definitely need a breast lift to centralize your nipple on your breast mound. If the implant is being placed above your muscle (which I do not recommend) you may get away with not needing a lift.

I would suggest a trial with tissue fillers and see how you liked modification of a few of your features. I always insist on three months after you stop breast feeding to do a final exam of the breasts and before I would consider surgery. A breast reduction/ lift with simultaneous implant placement is a common procedure, especially for those that have lost the fullness to their upper breast and whose breast glandular tissue has deminished and has been replaced by fatty tissue which does not hold shape as well.

The wide distance between your implants can be caused by capsular contracture, however your photos do not show a vsable contracture suggesting that you have grade one or two capsules. You have a prominent sternum and slightly sloping rib cage so there is a natural tendency for your implantss to fall to the outside whn you lay down. This can be improved with revisional surgery to open the medial (inside wall) of your pocket and suture closed the lateral (outside wall) of your capsule.

There are slight differences in the manufacturing processes for Mentor and Allergan, and if you examine the empty shells of the implants you will find the Allergan (Natralle) implants slightly thicker than the Mentor. Thus they offer more sizes with smaller fill variances than Mentor. They are both good implants when used according to the manufacturers recommended specifications.

Breast implants come in a range of sizes with the smallest implants being 125cc for saline and 100cc for silicone. Whether you will remain an a cup depends on your body size and amount of existing breast tissue. But the real question is "do you really want to stay only an A cup". I always inform my patients that they will get better results the closer they are to their ideal weight. But youy also have to be realistic whether you will actually loose the wieght. before surgery. In your case I would postpone your reduction until after having your next child as you are going to stretch out the breasts after your next pregnancy.
