Based on your description "sad and deflated", your assumed goals "perky" and "perfect", and your pictures, I think it would be difficult without raising your nipple/areola and tightening the skin of your breast which unfortunately creates scars. If you elect that option, post-operative aggressive scar management will be important. Make sure that your board-certified plastic surgeon discusses this with you.

Each of these surgeries are very often performed under local anesthesia, sometimes with a little oral medication to relax you pre-operatively. However, as you add multiple procedures, the limits of what you can do with local anesthesia become more apparent. You don't want this to be a traumatic experience. From what I see, I think you are an great candidate. While I agree that having a lower BMI would have optimal results, it is not always that easy to get it down. If you think that you can lose 20 lbs or more, its worth trying to do so first. Otherwise, you should just move forward with the surgery that should involve trunk liposuction to maximize contour improvement. Make sure you use a board certified plastic surgeon; safety is most important.

If you haven't received radiation therapy to the side that the expander was removed, it is possible that you could tolerate an expander followed by an implant again. However, your history of infection and the loss of tissue makes you at risk for complications. Using your own tissue will decrease that risk but is obviously a more complicated surgery and involved recovery. Your plastic surgeon knows you best and the recommendation is reasonable. Asking for other options or seeking 2nd opinions...
