The two issues in your situation are excess skin and the weakening of the abdominal wall.  These can be addressed with abdominoplasty.  If you hoping to have insurance coverage you will need a referral from your primary care physician.  It is important to understand that the prior surgical procedures, especially involving a hernia and a patch, can make the situation more complex.

I agree that sizing at a consultation is critical.  Rather than cup size, think in terms of the implant size, which is expressed in cubic centimeters (CCs).Remember that above 400 cc in size, implants can begin to significantly affect your muscles and bones.  Most of the patients we see with “J cups” are looking to get smaller breasts because of neck and back pain.

I saw your question on Real Self about the lower eyelids. I think the lower lid laser option would probably be best for the fine lines–I don’t think you require any surgical approach at this time. A little Botox works very well also in addition.

I agree that you will need a combination of skin removal and suctioning. My recommendation would be to have the procedure done in the spring sometime, and before then exercise as much as possible to improve your fitness for surgery and to get rid of as much excess fat as possible, so the surgeon can remove even more skin. There have been some good studies showing improved postoperative outcomes with preoperative exercise regimens.

The carbon dioxide laser is a big commitment with some significant downtime, but the results can be stunning. In general, it is hard to get a great result with minimal treatments. Lower blepharoplasty should be approached with caution in general and with extreme caution for the second time. Removing the fat through a transconjunctival approach (inside the lower lid) has helped with some of the postoperative lid problems, such as droopy lower lids. One thing not mentioned frequently in size discussions is the actual weight of the device. With a small frame, adding an implant in the 350 and up range starts to put a significant strain on your neck and shoulders. Almost always the implants are 400 cc or more. Very few less than that are changed. Subpectoral implants shouldn’t have a big impact on the function of the breast. You will have expansion of the skin envelope and will have to expect some increased skin laxity after childbearing and breast feeding. Pregnancy in general is not the best friend of your youthful body. Just remember that paying for your breast lift or tummy tuck is way less expensive than when your little precious decides to go to a private college.
