It is possible to fat graft the breast at the time of implant removal. There are many factors as to how much fat can be placed successfully in each breast based on fat availability and thickness of the residual breast skin after implant removal. From your picture it is likely a lift will be needed to remove exceed skin and reshape the breast.

Overall, good idea to keep direct pressure off the area for any extended period of time after the procedure. Your position is good but you may not be able to stay like that for long. Walking postoperatively is always advised as well. You should discuss this with your surgeon as well and refer to your postoperative instructions.

Smartlipo is an effective procedure that can safely be performed in the office by your plastic surgeon. It can be performed under local anesthesia and have minimal down time.

I do not see any reason why you should not be able to get shower at this point and remove your tape strips. This is likely a good time to start some scar care as well.

You will need to temporarily remove your piercings while the procedure is being performed but you or your surgeon can immediately replace them at the completion of the procedure.

Swelling is normal and to be expected after breast implant placement. It should reach the maximum by this time and slowly subside. The swelling should be resolved by 2-3 months at which time your breast size should be stable and you will then be certain of your new cup size.

A capsule is a soft, pliable thin layer of tissue with forms around all implants placed in the body, including breast implants. Having a problem with this capsule, capsular contracture, is rather uncommon in primary breast augmentation patients. If capsular contracture occurs, it can be conservatively treated but occasionally may require a second procedure for correction.
