Thank you for the question an I am sorry that the liposuction left you with loose skin. In my hands, the only way to fix the extra skin of your abdomen and thighs that did not tighten is to surgically excise it: an abdominoplasty or tummy tuck and a thigh lift. Those are the only procedures with enough power to really address that amount of extra skin. I would encourage you to see a Board Certified Plastic Surgeon to get an in person consultation and…

Thank you for your question and the photos and I am sorry that are having repeated episodes of capsular contracture. It can be a difficult and frustrating situation. I recently sat on a panel at our national meeting and all three experts were of the opinion that ADM may have some benefit. Your Plastic Surgeon is correct in that it is an expensive option. That said, in the setting of repeated episodes of capsular contracture, I use it as a last…

Thank you for your question and providing the images. The good news is that when I look at your pictures, I see that you nipple sits well above the inframammary fold (the fold underneath your breast). In that situation, you are often a candidate for an implant alone. Unfortunately, I don’t believe liposuction alone will take care extra skin. Liposuction could actually make it worse. By removing the fat with liposuction, the extra skin can hang…

Thank you for your question and the images. The lines that I can see in the picture are the nasolabial lines or folds (the line that runs from beside your nose down to the corner of your mouth). The best options are either fillers or surgery (facelift). Given what I can see in the pictures, you most likely do not need a facelift and fillers should work very well if done by a board certified plastic surgeon. Of note, there are lots of different options…

Thank you for the question and the series of pictures. You look like you would be a great candidate for Botox treatment of those lines. Your lines appear to be mostly dynamic meaning that when you are not flexing the muscles under the skin, the skin is smooth and even. When you animate or flex those muscles is when you are seeing the lines. By doing Botox now, you can minimize the risk that you develop static wrinkles (essentially wrinkles that are…

Without an examination, it is difficult to say for sure. What you are describing and what I see in the pictures certainly looks and sounds like you may have developed a capsular contracture. If you have not already, it would be a great idea to see your Plastic Surgeon as there are some non-surgical options that can sometimes help with this. There is some evidence that taking Singulair may help and there is an ultrasound machine (Aspen) that also has some…

Great question. There are certainly devices that are available to injectors now that help to make superficial veins more easily visible and hence easier to avoid. I have used the Accuvein and really quite liked it. However, it cannot be overstated that nothing will prevent complications. The benefit of these devices is that they can help to minimize the risks associated with injections and they work best of fair skinned individuals. Even the…

Thank you for a great question. In fact, many surgeons were interested in this exact question, so much so that a group has done a scientific study to answer it. It was published in the Aesthetic Surgery Journal. To summarize their results briefly for you; yes. They were able to show that in women that had a submuscular (under the muscle) breast augmentation, there was a significant decrease in muscle strength even 12 months later. That said, there…

Thank you for the question and accompanying image. It would appear that you do have a capsular contracture. If you are also having pain, then you would be classified as a Grade IV capsular contracture. Typically, this can only be fixed with surgery. It is a difficult problem with a risk of recurrence, so the surgery needs to be planned and executed with as much planning as possible to minimize that risk. I was recently on a national panel…

Interesting questions, thank you. Without images, it is a little hard to say how extensive the revision is. Most of the time, a tummy tuck scar revision is much less extensive than the original procedure. The key is to minimize the tension on the scar as tension is bad for scar healing. I would still typically have my patients minimize the amount of stress they put on the incision for 6 weeks after surgery to try and optimize the scar. That said,…

Your question is a good one, thank you. Yes, fat grafting can often be done at the same time as implant removal. However, there is a limit to how much fat can be transferred in one setting. It depends on the thickness of your own breast tissue, your extra fat (donor sites or places we can take fat from), and your overall health. In my opinion, it would be uncommon to be able to replace all of the volume you will lose by removing your implants (420…

Thank you for your question. It is less and less common for insurance to cover the removal of implants that were placed for cosmetic reasons. Even with an implant rupture, it can be difficult to get it covered. If your concerns over the ruptured implant is preventing you from getting your normal screening mammogram, then I would encourage you to pursue having them removed. It can safely be done as an outpatient and you will be back to yourself in a couple…

Thank you for the question and images. I suspect that both of those implants would fit your body quite well but will give moderately different results. The implants have a similar width (base diameter) . The difference is in projection. The 400 will give you almost an extra cm (1/2 inch) of projection (how much they stick out from the chest and 25% more volume (or roughly half a cup size more). In my practice, most women would chose the medium over…
