There are a lot of things that must be done to get an excellent result in Abdominoplasty. It is easy to criticize based on photos. Talk to your surgeon and communicate your concerns. He did the surgery and knows best what was done. If you are still not satisfied get a second opinion once you are healed. Revisions are never easy. Best to have a good discussion with your surgeon pre procedure about expectations.

What is offered as abdominoplasty varies widely. To get consistent high quality results is very difficult.The skill and experience of the surgeon is key. The procedure must be individualized. There are many factors that go into breast shape after augmentation. Sometimes the same volume implant looks dramatically different in one patient than another. Personally, I like the circumareola procedures. However I use them in cases where only a small elevation of the nipple/areola is necessary and a large augmentation is not desired. The implants in that situation is used to balance the tendency for the circumareola procedure to flatten the breast. Usually when implants are replaced a somewhat larger implant is chosen to fill the skin envelope. The skin envelope is not likely to shrink enough to give a good result w/o a good deal of sagging. Usually a skin reduction procedure, a mastopexy, would be performed to reposition the breast and implant. It then could be placed in a sub pectoral plane. Usually I try to put the new implant in a new pocket. This likely was not possible w/o a mastopexy. Best to discuss the options…

II have seen patients on a weight loss program diet down to an unrealistic weight before surgery only to gain again when they abandon their program after surgery. Liposuction is not weight loss. It takes time for the scar to mature, Often there are absorbable sutures that need to go away as well. Sometimes the blood suppy is not what was expected and there is some fat necrosis which sounds like what you had. Your surgeon knows best what he did in the procedure and what should be expected. Now is the time to trust him to do the right thing for you.

You need to be very cautious in evaluating Pre and post of photos. Even at meetings we need to compare things very critically. Photographic technique and manulipation can make a surprising difference even when no attempt is made to deceive .

Also, I believe most reputable surgeons will post “representative” results. Middle of the road instead of the one in a hundred. I think it is important to give patients a realistic view so they see the scars and trade off of the procedure…

Abdominoplasty is a complex procedure. There are many options to accommodate various patient needs. The differences are often not appreciated even by some plastic surgeons that do not do a lot of abdominoplasty. Personally, I always use liposuction in abdominoplaasty and vary my skin excision based upon the patient needs. The skin excision can be designed to greatly improve the waist area. A complete plan of attack pre-operativly is indicated.

Personally, I am not a fan of no drain/plication techniques. I think I have better results and fewer complications with drains. That said a lot of good surgeons have reported good results with this technique.

You need to talk to your surgeon. If you are not comfortable ask him to get a second opinion. Sometimes an ultrasound is helpful to locate a collection but remember aspirations carry their own risks.
