Lower abdominal tattoos can be removed surgical with a tummy tuck
In a tummy tuck the skin of the lower abdomen is removed. Usually the skin excision involves an ellipse of skin that goes horizontally from hip bone to hip bone and vertically from the belly button to the pubic bone.
If the tattoo is within that area it will be removed with a tummy tuck. Unfortunately, most tattoos that I have seen in the lower abdominal/groin area are positioned so that only the upper half of the tattoo can be easily removed with a tummy tuck.

Tatoos and Stretch Marks Can Be Removed With A Tummy Tuck
One of the less commonly discussed benefits of a tummy tuck is that it is quite effective at removing stretch marks, scars and tatoos of the lower abdominal region.
Because the skin is removed, any one of these can be removed as long as it is located below the belly button, in or above the hair bearing pubic region and between the hip bones. The final scar is usually hidden by underwear or swim suits and does well long term.

You may want to post a picture to help answer your question. However as long as the tattoo is located between your umbilicus and the pubis there should be no problems removing the tattoo in conjunction with the abdominal skin and fat.

The incision for a tummy tuck is often placed upon the lower abdomen, near or at the hair-growing skin of the pubic area. The tissue removed typically is the skin and fat from this lower abdominal position, up to (and often above) the level of your umbilicus (belly-button).
If your tattoo is somewhere between your belly button and pubic region, then it can likely be removed during the tummy tuck.
