You can have the extra skin removed and most likely need a full horizontal and a vertical incision all the way to the top of the abdomen to get the smoothest result. You have had a massive weight loss, and this has left a lot of excess skin. It might be possible to get a reasonable result which you will be happy with with a traditional tummy tuck which leaves you with a scar across the lower bakini line.
However, it would probably leave you with a lateral excess of skin which is not corrected by this procedure. Some patients require an additional vertical scar in the middle of the stomach to remove this lateral excess.
