If 405cc’s are too big, I would think that you would want to be at least in the mid 300’s or in the low 300’s. Of course this can only be determined after examining you in person and discussing it with you. The surgery itself is usually very simple, unless the pocket needs to be revised. Again, examining you in person will be necessary to determine this.

Even though i agree with the plastic surgeon who said that you have to be careful about the quality of “cheap” plastic surgery, the prices you quote do seem very high. The total price for a mastopexy-augmentation and tummy tuck together is more in the neighborhood of $10-11K in this area.The cost of living and doing business is lower here in Tulsa, so I would expect prices here to be lower than in other urban areas.

When looking at the cost of any cosmetic surgery, there are usually 3 components: facility fee, anesthesia fee, and surgeon’s fee. Currently in my practice, when taking all of these three fees into consideration, the cost of a tummy tuck is between $6000 and $6500. These surgeries are performed at a surgery center or hospital operating room. If an overnight stay is desirable, there is a charge of $425. We participate in Care Credit financing as well.

To my knowledge, no one knows exactly how much pectoral muscle exercising you can do without displacing the implants down and out. I agree with your priority of putting your profession first. If you are training to get in shape, and not primarily lifting weights like a body builder would, the risk of displacement is probably not as much. I agree with the other doctors in wearing a supportive bra during training and work.

Although it is very difficult to give you a definitive answer just looking at your pictures and not examining you in person, it appears you would benefit from an implant and a lift. As far as an implant alone without a lift, the usual thing you look for is the nipple level compared to the fold under the breast. I agree with most of what the other doctors have told you. The traditional management for an extruded implant is to wait at least 3 months and then re insert the implant. It will definitely look different than the other breast at first. The final outcome will depend on the quality of the tissues on the side of the infection.

I agree with the other doctors that just having implants will not look attractive. It is difficult to tell with certainty because your nipple position is covered and the photos are limited. A physical examination would be necessary. However, the patient you show with the desirable result has breasts that are very much different than yours. Her nipple position appears to be much higher. Your breasts are also situated lower on your chest. I think that a lift will probably be necessary.

From looking at your pictures, it appears to me that insertion of implants alone will round out your breasts sufficiently without the external scars of a lift. In general I place implants under the muscle in your age group, but occasionally if i need to alter the shape of the breast significantly I will place them over. One would have to examine you and discuss these options with you to choose the right plan.

It is not unusual to have some degree of bruising after a breast augmentation. To have enough bleeding to warrant a return to surgery for removal of blood clots (evacuation of hematoma) there would usually be major swelling or enlargement of the involved breast. In your picture i cannot see the other side, but if the breasts are relatively symmetrical, then a wait and see plan is not unreasonable. I would have to examine you to know for sure. It appears from the picture that you have excess lower eyelid fat. It is difficult to tell whether there is excess skin. I agree with the surgeon who told you that only your surgeon can instruct you regarding post op activity. However, i can say that it usually takes 6-8 weeks before you would feel comfortable enough to perform very strenuous and demanding activities. Don’t do too much too early.

I usually perform liposuction at the ends of the incision of a tummy tuck to reduce the tendency to have a “dog ear” or bulge at this point, and to improve the contours of the flank areas. The pubic area is also frequently suctioned so that it doesn’t stand out under the tight lower abdomen. The amounts of fat removed are usually small, and the risks are much smaller than compared to treating larger areas (hips, thighs).
