Full exam is necessary. An option is a sub periosteal mid-face (cheek lift). This involves surgically lifting your entire cheek, vertically. This method reversers the vertical drop of your cheek soft tissues that causes the Nasolabial fold to get fuller and the cheek bone to look flatter due to the aging process. After a complete exam for gynecomastia I typically recommend a combination of excision of the breast tissue and liposuction of the chest. This approach will remove the breast tissue that is under the areola and the liposuction will smooth out the chest contour after the breast tissue is removed. On very thin males, maybe just the breast tissue excision is adequate.

The brow lift improvement can be simulated by looking into a mirror and lift your brow upward. You will probably see improvement and may still benefit from the upper blepharoplasty. However, depending on your physical exam, the upper blepharoplasty may be ok. With either operation I typically recommend removing the frown muscles.
