I’m not sure infrared therapy will help with the scarring. If anything it might produce hyperpigmentation of the scar. There are other modalities for improving scars that have been proven to work. Silicone strips with overlying pressure seems to work in most cases.

The abdominal muscles are not normally cut during an abdominoplasty, they are approximated or imbricated to approximate them to the mid line. This results in a flatter abdomen.

MS by itself is not a contraindication to perform surgery. It depends on the severity of the MS and a surgical clearance is definitely needed before the procedure is done. It may be prudent to do them in separate settings so as not to overwhelm you.

You have two problems. That of a recurrent abdominal hernia and a prominent abdominal pannus. You need to have the hernia repaired and maybe an abdominal wall reconstruction. Once this has been taken care of, removal of the pannus can be done.
