I don’t feel that you truly have a tuberous breast. Shape like yours is not really that uncommon. You should be able to have a very result. I would likely recommend a small lift with your augmentation that will allow for breast shape improvement and achieving the size of your choice.

It is always best to be at or near your lowest weight before having a tummy tuck. It will certainly reduce your risk of complications, and it will result in a much better result from your surgery. I don’t usually give patients a certain weight that they must get to, but it is important for you to reduce weight in order to make a tummy tuck a safe operation for you. You do not have a typical tuberous breast deformity. Your skin is very tight on the lower pole of your breast though. I would recommend just simply a breast augmentation, but I would caution you to not try to go too big. You can have a very nice outcome starting from where you are.

I frequently combine procedures to allow my patients to have recovery at the same time.  I usually tell people that I would try to limit the length of surgery to 6 hours or less if you are healthy enough to tolerate it.  It would require a consultation to determine if you would be a candidate to do all of that at the same time.

You have some volume asymmetry of your face.  This is very common and present in most people to some degree.  Improved symmetry could be achieved by adding volume to your right side with fillers or fat grafting.   That is a very common and simple treatment.  There is also some asymmetry in your smile as you mentioned.  I don’t think that you necessarily need to do anything about that, but if it bothers you it could be treated by injecting some botox on your left side to slightly weaken it to…

Implants always have the possibility of displacement after surgery.  The risk is reduced with careful dissection of the pocket, and use of smaller implants.  The use of support bras after surgery can also help reduce this risk.  You have bottoming out of the left side.  This can usually be repaired with some internal stitches (capsulorrhaphy).  Occasionally additional support needs to be added with an additional product sutured in to add support.

It is very important that you discuss your wishes in detail with your surgeon.  I like for patients to bring in photos of things that they like or don’t like so that I can have a clear mental picture of what they want.  It is much easier to make a lower incision in some patients than in other, but I think that you would likely be able to have that.

If you are not satisfied with your results then there are things that can be done to improve your results.  Your belly button could be made smaller with better contour by doing an umbilicoplasty.  It also looks like you may benefit from some liposuction of your upper abdomen.  It is not uncommon to not do that at the initial tummy tuck, but it can safely be done now.  You should discuss this with your surgeon, or seek out a board certified plastic surgeon for another opinion.

General surgeons are very skilled at fixing hernias.  They rarely treat distasis recti.  Muscle separation without a hernia is usually not covered by insurance.  This falls well within the common practice of most plastic surgeons.  We commonly repair diastasis during nearly every tummy tuck that we perform.  I would recommend that you see a board certified plastic surgeon to be examined and get the best advice for repair.
Does alloderm and strattice help eliminate capsular contracture?

I have had a very good experience using Alloderm to treat the most difficult cases of capsular contracture.  I have not had any recurrent contractures when I have used this to replace / prevent capsule formation.  It can be costly in cosmetic situations, but it has a very good chance of preventing future contractures when it’s used.
