For better or worse a tummy tuck won’t change your metabolism so you will still gain/lose weight at the same rate you did before the surgery. You are correct that if a large volume is removed from one area of the body that area will be more resistant to changes in size where other areas will pick up the slack. I think the “blood bag” you are talking about is actually a JP drain. It is used to get rid of the fluid that your body makes after a tummy tuck. Most of the time right after the surgery that fluid has a little blood in it which makes the fluid red. The drain has a suction bulb at the end that collects this fluid so that it doesn’t make a mess. As you heal, the body makes less fluid and the drains can be removed.

With the information you gave and these pictures, this is a pretty easy one for me (I’m a board certified plastic surgeon). Loose the weight as the first priority. Progressive tension sutures are typically dissolvable. They are sometimes referred to as quilting sutures and act to reattach the fat layer to the muscle layer after a tummy tuck. They are generally used to take tension off of the lower incision as it heals and reduce the chances of developing a fluid collection called a seroma. From a biology standpoint you could have the surgery but there may be some benefits from waiting. First of all you want to be back to a stable weight and if you gained weight during your pregnancy and are planning on loosing it you should do that first. Second you need to consider the baby. This a great question that I have been asked more and more lately. Not every patient needs to have their abdominal muscles tightened. It is true that if you want the flattest tightest results than you would want to have the muscle included in the repair but if your muscle only bulges slightly then it may not be completely necessary. 6 weeks is usually the standard answer when considering how long it will take the tissue to recover under normal circumstances. Other things to consider is if it is safe to under go general anesthesia two times so close together as the risk for a blood clot in your leg (DVT) goes up when you are put to sleep. Check this with your plastic surgeon and they will probably complete a survey called a Caprini Score that will help figure out the safest course of action in regards to preventing a DVT.

While being in good shape before a surgery is always a good idea, doing abdominal exercises is not necessary before having a tummy tuck. Patients who have a healthy lifestyle tend to recover faster so I always encourage this but it doesn’t affect a surgeons ability to perform a diastasis repair

The clover will be removed but I think your floral pieces should be safe. In regards to your question about only having a mini-TT, I don’t think that’s a good option for you. It looks like the reason your stomach sticks out is because the muscle layer underneath is stretched out and needs to be repaired. That would require a full TT.

Liposuction with a tummy tuck has been shown to be safe but it can increase certain risks. Liposuction can decrease the blood flow to the skin so if it is done aggressively or in areas that are providing a lot of blood flow to the skin it can result in healing issues or cause some of the skin to die. For this reason when liposuction is done during a tummy tuck, it is done very judiciously. Also, liposuction can sometimes result in the body making more inflammatory fluid after the procedure….

It is totally normal to be nervous (I know I would be a little and I’m a plastic surgeon!). That being said awake chin liposuction is one of the easiest/least painful things out there. Because the area is small it numbs up fast and then the liposuction part is over fast. When it comes to liposuction the more fat there is in a particular area, the more volume can be removed and the greater the change that can be created. In these photos it doesn’t look like you have a lot of fat so I would be surprised if liposuction created the kind of results you are looking for. That doesn’t mean that there aren’t option though. The general teaching in plastic surgery training is that most drains remain in place until the output is consistently less than 30ml. The reason you don’t want to remove a drain too early is that the body has trouble absorbing large volumes of fluid and you don’t wanting the fluid accumulating under the skin and forming a seroma. There are various ways to reduce your chances of developing a seroma, one of which is when a surgeon uses progressive tension sutures.
