Yes you can get implants placed with autologous tissue . But , every case is patient specific . What Flap was used? What occurred in each of the surgeries? To much missing info to give exact advise . Once your incisions are healed you should be able to work out . The concern I have is always open wounds and sweating or direct trauma . I f you had a reconstruction for breast cancer you probably have an implant behind that nipple . So, be safe and wait till your healed .

At 4 weeks I do not think you should be worried about appearance . Follow your surgeons post op protocol and what at least 8 weeks . Let the swelling get out of the tissues than evaluate . And with out knowing the technique used it is hard to comment more. Nipple sparing IMF incision ? What did you look like pre op to assess were you are now ? I would use the same incision to remove the implant . Laparoscopically removed with full exposure . As for the capsule it would depend on the character of the capsule. If the capsule is diseased the I would remove it . But you could leave it ,score it to close it down etc . I would plan for removal and intra-operatively make the decision of what is right .

With a narrow chest and a 12 cm base with those sizes sound right . Elasticity of the of the skin , damage to the skin with stretch marks all will play into the need of a moderate plus vs a high profile . With a narrow chest the M+ may be to wide and a HP will work better. It also depends on what look you are looking for. More projection with narrow cleavage, over vs under the muscle etc.

The only way to get rid of extra skin is with a tummy tuck. No lateral views to the the abdominal muscle . If bulging then you need the muscles fixed with a plication.
