I would suggest continous silicone scar sheet application starting two weeks after surgery for three months plus monthly evaluations by your plastic surgeon for potential Kenolog steroid injections and/or laser therapy.

Healing will take about 3 months, when managed properly.

It will take several weeks until the extent of the necrosis has defined itself.

Debridement should be done only on clearly necrotic tissue.

Frequent visits with your plastic surgeon are key.

Any revisional surgery, if needed, should wait at least 1 -2 years.

Excessive resting in bed may promote blood clots.

Excessive activity might start up bleeding.

The middle ground, taking it easy and resting the first week is prudent – the body deserves time to heal.

At six months after your last surgery, I would suggest releasing the lower aspect of your implant pocket on both sides – to lower the implants. The areolar approach would be most suitable for this.

Also, your new implant volume seems disproportional for your frame. You may wish to re-consider a smaller size to end up with a more natural appearing result.

The description suggests an oversized implant pocket out towards the side.

At this point, would suggest external taping and a supportive bra for several months to keep the implant centralized behind the breast while the peri-implant capsule is still forming.

It this proves unsuccessful, the eventual capsule may need to be surgically reduced  (Lateral Capsulorrhaphy) to match the implant,  about six months after the initial procedure.

Since you are able compare left to right, you should have the right side examined by your surgeon for normal healing – perhaps there was a small area of separation.

Once cleared to begin your implant massage, do so for the next three months, morning and evening, and present for monthly follow-ups to monitor for signs of early contracture formation.
