It looks like your chest is still recovering from surgery and the added trauma of a hematoma evacuation procedure. I would give the skin about 6 months to tighten up and smooth out before looking into any revision procedure. Hopefully, you wont need one at all!

The short answer is “yes” but it depends on a number of individual factors, including BMI, amount of fat vs. skin, location of fat amenable to liposuction, and overall tissue quality, In some circumstances, I recommend staging the procedure, so that liposuction is done as a first stage procedure, followed by thigh lift about 3 months later. Photos or, even better, an in-person or exam, can help determine the optimal treatment plan.

My practice is to communicate directly with the prescribing provider to ensure that all three of us have a concrete post-op pain management plan that keeps the patient on track. This is a common problem and frustration. That’s extra skin, so more liposuction will not help. My preferred treatment is an upper body lift/bra line back lift. When done correctly, the operation leaves a scar across the back, but is strategically placed behind the bra strap. It’s a very powerful operation and is a surprisingly easy recovery with low complication rate.

You’re a great candidate for abdominoplasty, but your result will be significantly limited if you don’t have the muscle tightened. You have significant muscle laxity, especially in the lower abdomen, which give you that lower abdominal prominence that bothers you. Prior liposuction won’t be a problem. It doesn’t look like you have significant contour irregularities from prior lipo so I expect your result to be smooth and flat.

It’s normal for implants to start high and gradually descent into position, to fill out the lower pole. This process typically takes 6-12 weeks. If you’re 6 months post-op, you probably need a revision/capsulotomy to drop the implants, especially on the left.

Yes, I think you are a good candidate for breast lift with implants. Breast lift technique varies by surgeon (crescent vs. circumvertical, etc.) but the point remains that large implants with a small lift will be a good option to achieve your goal.

It wouldn’t be wrong to just do a blepharoplasty for the reasons that you mention. It will address that sagging skin on your upper lids. However, it looks like your brows have dropped a bit. Presuming this isn’t a result of Botox (or other neurotoxin) which can cause the brows to drop, a brow lift would be great as well. But certainly not necessary.

Your muscle needs to be tightened substantially, which will tend to bunch the skin up in the midline.In order to flatten that out, the skin will need to be re-distributed over the entire surface of the abdomen, necessitating a full-length incision. You are a great candidate and I think you’ll have an awesome result.

Your implant is bottoming out a bit. I would offer you a revision but whether you should do it depends on how much it bothers you. It will not get better on its own, and will likely get worse with time. However, it’s not urgent so it can be done when it works for you.

I place 2 drains during tummy tuck. Typically, the first drain comes out after 1 week and the second comes out 1-2 weeks later. Timing of drain removal depends on how much fluid is coming out of the drain, as well as the color of the fluid. Depending on your anatomy and how much fat is surrounding the gland, most surgeons will do gland excision with some liposuction to contour the chest and avoid the uneven contour you’re describing. Most surgeons hide the incisions at the border of the areola. A scar underneath the areola is only necessary when there’s a lot of extra skin, namely in massive weight loss patients.

Gland excision will work well for you. It will reduce the bulk behind your areola so they’re not as prominent. The areolas will shrink when they’re not being stretched by the underlying gland.

I think a breast lift with/without augmentation is achievable at your current weight. Every patient’s weight distribution is different, so it’s hard to say without seeing you. However, in general I counsel all patients with a BMI over 30 (yours is 32) that their risk of complications is higher and quality of results will be limited. If you’re more “pear shaped,” however, you may be good to go at your current weight. I think it would be worth seeing a surgeon for an evaluation in person. Enjoy!
