Hi there. Based on the photo, I don't think that use of adapalene will give you the results you desire when it comes to clearing up these old acne scars. It may be helpful to discuss options with your dermatologist which may include micro-needling or laser therapy. All the best.

Hi there and thanks for your question. What you are experiencing is a very common side effect of tretinoin use. To speed the healing of the red flaky areas, use liberal moisturizer (make sure it is a non-comedogenic moisturizer). It may be helpful to use the tretinoin once every 2nd or 3rd night at first to limit the red/flaky side effects and increase the frequency as tolerated. You can also inform your provider and consider a lower strength of the medicine which may be less irritating.

Congratulations on the successful treatment of your acne! Generally, when your provider prescribes isotretinoin (Accutane), they have a certain goal dose in mind that they would like each patient to achieve. I would discuss with your practitioner to decipher if you received an adequate total dose before stopping or simply complete the course as prescribed.

Hi there and thank you for your question! For thicker keloids, we usually recommend steroid injections into the keloids to help soften the scar tissue and reduce any symptoms the keloids may be causing. I would not expect tretinoin to result in much improvement of the keloids. I would recommend in-person evaluation by a dermatologist to further discuss treatment options.

If this area has been thinning out over months-to-years, it may be wise to invest in over-the-counter minoxidil 5% therapy to help combat the hair loss in this area. If things do not improve to your liking or continue to worsen, it may be helpful to visit your local dermatologist for further evaluation and discussion of other treatment options.
