Your picture appears to show descent of malar fat pads. Perhaps your dissolving of the filler unmasked the natural aging of your face. So your choices are to refill the upper cheeks to mask/camouflage the descent, or to consider a facelift or mini lift. Which means you seem to gain weight more easily. But it is not that you simply gain weight, it is still a question of taking in more calories than you use. Unfortunately, as I tell my patients, and have suffered myself, being overweight is much like being an alcoholic. To be sure your healing is progressing well. Remember that in plastic surgery, final results are usually a year in developing, certainly not in the acute phase of healing (the first six weeks). There will be lots of changesin the looks of your nipples and breasts as the effects of surgery calm down.

As is some depression. You should wait at least a few weeks, and discuss it thoroughly with physicians and other breast augmentation patients to figure out what is best for you. Any big change to your appearance is going to involve episodes of ‘this is not my body or me’. That is normal. Give yourself, and your healing body, a little bit of time to make your final decision. That is, the glands in your breasts grew in preparation for breast feeding. They did this slowly throughout pregnancy. So it is reasonable that it would take at least 6-12 months for those changes to disappear or ‘regress’. Congratulations on your pregnancy!

From all of the changes of pregnancy yet. Moreover you are carrying some extra weight. It took you 9-10 months to develop all of the changes and deliver the baby. Your breast glands, and frequently your uterus, have not completely involuted, or shrunk back down to prepregnant state, yet. 4.5 weeks is much too early to assess the results of plastic surgery. Your body will take several months to calm down, and will keep ‘remodelling’ for the rest of your life, just as it would have if you had not had surgery. In general, one would wait at least 6 months, possibly a year prior to revisions, to give scar tissues inside and out time to mature. Look for a board certified plastic surgeon with experience in that particular area of fat grafting, many of us fat graft to breasts, buttocks, contour defects and faces as well as hands. There are multiple options for making chest walls appear more symmetrical, as there are for many parts of the body. This could involve implants, fat grafting or more extensive surgery. All of this of course would be dependent upon underlying anatomy and physiology. Good luck!
