The active ingredient of Ozempic and Wegovy is semaglutide. Wegovy goes to higher doses and is FDA approved for weight loss. Rybelsus is the pill form and is generally thought not to give as strong appetite suppression. Mounjaro acts on 2 receptors and I think is best reserved for people with insulin resistance or diabetes type 2.

I have many non-obese patients who have taken semaglutide to lose 10-20 pounds. Once they are stable at the target weight I wean them off. I have patients who are stable at that target weight for over a year now. It seems they have achieved a new set point. I think if a patient is starting at 50 pounds or more over their normal weight, I like to keep them on a maintenance dose longer because their chances of regaining the weight are higher.

When someone loses weight, by any method, all fat cells in the body will shrink. If weight is gained they get larger. The same goes for fat grafting to the face. I would recommend doing it when you are close to your target weight.

I love micro internal ultrasound for neck lipo. It liquifies the fat, making it easier and more safely remove the fat. The heat it creates also stimulates collagen in the skin and on top of the muscle which leads to skin and muscle tightening.

Most of my patients have neck liposuction with local anesthesia. I give a little muscle shot for sedation so they are relaxed. If someone is very anxious or a control freak I recommend they have sedation or twilight anesthesia. They will not feel anything and just wake up when it is over.

In general the layer of fat in the neck is thin, so using a thinner cannula is best. I like to use internal ultrasound which breaks up the fat and tightens the skin. This is 1.8 mm thin. I then remove the fat with a 2.0 mm spatula cannula, which only has the suction on the underside so a thin layer of fat is left just under the skin.

You definately have a small chin but it does not look like you have much neck fat. I would consider a chin implant for the most reliable projection. Any little bit of fat under the chin will not be seen as much.

Your chin looks a little small, deflated and asymmetric. Fat grating is a great, natural option to strengthen the jawline and it will also camoflague the little jowls you have, as well as, improve the symmetry.

It looks like you have a good amount of excess neck fat. The chin projection looks good, but there are little notches on the sides of you chin (pre-jowl notches). I would recommend filling them with some of your neck fat.

Yes, while it is a variation of normal, it is large. It also looks like you have a constricted breast and ptosis (droopy breasts).

No drift with fat grafting. It is permanent movement of fat cells, so if you gain or lose weight in the future the area will get bigger or smaller. I do not like fat graft to calves, implants are a more reliable and powerful solution.

It sounds like you would benefit from lower eye lid filling. I like hyaluronic acid there because lasts at least 2 years and looks very natural. I do not like fat there because the lower eyelid skin is so thin, it is possible for fat to look too bulky or see little nodules.Fat is great for the cheeks and looks natural there. Adding some volume to the cheeks also creates a lifting effect to the face.

The implant appears a little large for your frame and you are right the breast looks like it is falling off of the implant. I think you may benefit from going down a bit in size and revising the lift.

It looks like you may have had tubular or constricted breasts prior to surgery with uneven breast folds. The dent is either from lowering the breast fold and/or if the implants were placed under the muscle, when flexing, the dent is accentuated. A revision surgery should improve the appearance, but may not fully solve the problem. Also slightly larger implants may help to fill out the area more.
