You would be an excellent candidate for an abdominoplasty. Almost all people will benefit from liposuction of the torso too as part of the procedure. Many surgeons are uncomfortable performing larger volume liposuction at the time of an abdominoplasty because either their surgical facility is not set up for longer perioperative recoveries, time constraints, or fear of adding complication risk. I routinely perform 3-6liters of liposuction with abdominoplasties always in a hospital setting....

Excising all breast tissue under the nipple will often result in a depression. It the depression can be grabbed and pulled upward indicating it's not tethered to scar tissue. If so fat injection or filler can be injected to fill the depression. If the depression is fixed then using the existing incision to release the scar will then allow fat injection to work better.

Gynecomastia implies breast gland is excess not just surrounding or associated excess fat. Breast gland is firm and rubbery and can not be suctioned. This breast gland tissue is easily removed with a small incision at the lower half of the nipple. This can often be done under local anesthesia.

You do have exceptionally deep naso labial folds. Facelifts can work well for the lower half of the fold but don't usually improve the upper half of the fold towards.the nose. Fat injection under the fold does a poor job of "cracking out" the fold and oftens just lifts.the fold up without reversing it. Fillers in the central and lateral cheek either fat or hyaluronic gel work well. Filler under the fold after a a facelift will help. Repeat the filler over a year and again can permanently...

Patients are fascinated by necklift incisions that are only behind the ear. However when the excess skin is lifted and excised behind the ear it severely limits the lift power and direction you want it to go. Also when you ask a small incision to do a big job you often get a short scar but a poor quality one with pleats of skin and thickness. The facefilt scar portion in front of the ear is usually better quality than the one behind the ear and often is just a very faint white line.

You are young for a facelift but that's okay. A mini facelift and Smartlipo of the neck done under local anesthesia with just oral sedation would be a great procedure for you.

The term "full facelift" is confusing because that often means multiple procedures. For example a brow lift, upper and lower blepharoplasties, and fat injection are separate add on procedures. It is amazingly easy to numb the face neck and eyes in awake patients so they don't feel any pain at all during surgery. This numbing is used even when patients are totally asleep under anesthesia. I do 90% of my facelifts under local anesthesia with just oral sedation. Patients are awake and talking ...

Aging in the face is not due to muscle sagging. Muscles stay where they are. The overlying fatty tissue and skin is what sags. Just like doing situps won't change abdominal skin laxity stimulating face muscles also won't lift fatty tissue and skin. The platyma neck muscle is different than most and can become lax. However very little to no improvement would be expected without a face or neck lift.

You are 28 too young for a facelift unless you have had massive weight loss. Cheek implants are smaller than you think. The tissues will.contract to their new smaller volume to some extent over a year. Hyaluronic gel filler would be a great option that is safe, long lasting, and can have permanent improvement due to tissue remodeling from the previous injections.

If you don't like your filler fortunately you did not inject anything that's permanent like silicone. Hyaluronidase is an enzyme that breaks down hyaluronic gel usually very efficiently when injected into the filler. Perhaps you need more hyaluronidase. The best option is to do nothing and let it resolve to normal over the next year. Surgery or liposuction can't remove it.

I think you're too young for a facelift. The scars are never invisible and I don't think you want to others to see them. Juvederm to the nasolabial folds and possibly Voluma to add some contour to the cheeks would be a simple way to improve those areas without surgery.

You have significant ptosis. An implant below the muscle without a lift will result in high appearing implants and breast falling off the implant. Implants on top of the muscle will look less strange without a lift but produce the problems of subglandular implants. I would recommend a T scar lift with Mentor HPX Implants in the 300 size. The T scar is slightly more scar than a lollipop but the scar that matters the most which is the nipple and vertical portion are usually the best quality...

Using a larger implant to avoid a lift will often leave you with a size you don't want and still have ptosis. If you are absolutely against any lift scars an implant on top of the muscle can make the breast larger and follow the droopy breasts with it so you don't have as much tendancy for a high implant with breast falling off it like a submuscular implant. However implants on top of the muscle tend to have many more problems over time. I would suggest an implant volume you like with a T...
