Have a discussion with your PS about what an ideal B.B. is to you. I prefer a slit like one but you are correct in the obvious scar takes away from the results. Scars are still obvious at 8 weeks so be patient. Your surgeon may need to shorten the stalk which will pull the scar down and out of view.

I know you are concerned and hopefully this will improve with time. A partial submuscalar implant is the key to most larger augmentation. Do not ever do heavy pectorals work in the gym as this will worsen the issue. Wait at least 6 months than reevaluate the animation issues, fold position, and even implant size to determine if your results are reasonable.

This is certainly a tough problem for patients and Plastic surgeons to fix. Perfection is usually not the goal but sometimes we can get pretty close. It’s difficult to say without knowing what your surgeon did in regards to internal tissue scoring, etc, but at this point, you look about right. Be patient, and stay off social media while in recovery. You’ll drive yourself crazy and will get mixed opinions which you don’t need at this point. Hang in there.

When the shaped texted implant first appeared in the US, many of us believed the hype and used them often. We have come to learn that they are a bit to firm and unnatural as well. Other problems we see is that they can flip over and lead to major distortions. Not to mention potential problems with textured implants and ALCL. Smooth round gels are the best!

A consult is the obvious answer and make sure it's a board certified plastic surgeon with experience. Many factors need to be considered including your skin quality, future pregnancies, chest wall characteristics and cost. A moderate/large implant partially submuscular will correct most of your issues really but the areola will be stretched. If you desire a smaller increase in size, a vertical lift may be better.

After years of trying to make a limited mastopexy work, most of us have come to the realization that it rarely is as effective as we'd like and often times leaves an ugly wide scar. You are to be congratulated on your weight loss and lifestyle changes. Keep it up. Loose skin in my opinion like yours needs to be surgically removed. Anything less may compromise your ability to have skin removal later. The procedure you described will not effect the skin below that on your lateral breast/chest unfortunately. I often will include that in a brachioplasty but in most offices that's an extra charge. Make sure you discuss this with your doctor.

Radiation is the real culprit here even though I would have taken out the majority of the Alloderm as well if not all. Alloderm has to incorporate and become part of your native tissue, which is compromised by any number of factors including radiation. I would recommend against pain management and work with PT and excercise. You will be a candidate for reconstruction again if your cancer remains stable. Hang in there☺️

Any medication with pseudifed in it could have detrimental effects on blood supply and healing with a MM. This medication causes vasoconstriction which slows blood flow to tissues that are tight or stretched as in TT and mastopexy. I've seen this in several patients that thought it wasn't important to mention pre-op and ended up with tissue loss, etc
