This scar has two issues. The first being the hyperpigmentation ( darkness) over the scar. The pigmentation can be at multiple levels in the skin. It is very normal for a scar to  be intermittently swollen for months after the initial surgery, especially in an area that is constantly moving. ie. neck. If the scar continues to swell or causes you pain, please see your original surgery. You may need a scar revision ( removal of the dog ear) or a cortisone shot

Botox and Tretinoin are very safe when used together. A retinoid , i.e tretinoin , is an important part of your skin care regimen. Apply a small amount to clean skin at night and follow with a moisturizer over it. Initially, start every other night. If you have sensive skin begin every third night. Microneedling is a good option for stimulating collagen. The combination with topicals ie hylaronic acid, vitamin C, retinoids, peptides... gives the best results. Microneedling is excellent on the rhytids (wrinkles) above the lips but should not be used on the lips themselves. Remember to take an antiviral prior to the procedure if you are prone to cold sores.
