There are a variety of ways to close a tummy tuck incision. This varies form surgeon to surgeon. I usually close my tummy tuck incisions with absorbable sutures that do not need to be removed. Talk to your surgeon about how /she plans to close your incsion. Let them know that you have an allergy to staples.

Generally speaking, recovering form a breast lift is easier than other cosmetic procedures. There is little discomfort that is often controllable with tylenol. You may or may not have drains; I usually do not use them for a lift.

All surgical procedures have some risks. Combining two surgical procedures such as a breast lift and a tummy tuck can increase risks associated with bleeding, infection, anesthetic risks, and deep venous thrombosis, among other things.

These implants can give you a more fill above the nipple line while providing a less projection. Usually in a re-augmenation with a significan capsular contracture, I prefer to use high profile implants that are bigger than what is present. Because it is difficult to judge the right size implant after a capsulectommy, I usually use sizers to help select the best implant size.

You sound like the typical patient I see. If your dimensions are in centimeters, then it is unlikely that you would require a lift. Without looking at your breast tissue it is hard to estimate an implant size. However, I usually prefer either the moderate (Allergan) or moderate + (Mentor) implant for a primary augmentation. After the implants are removed, the breast skin is stretched and may or may not contract. A mastopexy may need be performed in order to tighten the skin. Placing the implant above the muscle may give more of a lifting effect, but only in patients who have enough breast tissue. Your areolas appear a little low and large... you may be a candidate for augmentation with a periareolar lift.
My best advice to you is to seek consultation with a board-certified plastic surgeon...

I have this conversation with all my patients. Naturalness is in the eye of the beholder. Generally speaking, the smaller the implant the more natural it looks, but because the breasts aren't' much larger than what you started with. I always ask the pant size my patient wears as I try to establish harmony between the breasts and the hips.
Your surgeons plan appears typical. My preference would be to use the moderate (allergan) or moderate plus implants (mentor) but that is a surgeon...

It is unlikely that the excess flab will affect the umbilical hernia. An umbilical hernia can be repaired at the same time as the excess skin removal during a tummy tuck. I doubt if your excess skin will be covered by your insurance. Your hernia repair is usually (but not always) covered by insurance.

Diabetics generally have suppressed immune systems and less ability to heal wounds than non-diabetics, especially if the diabetes is not controlled. I am not aware of any specific studies that compared complication, infection and wound healing rates of diabetics and non-diabetics for this operation. I counsel all my patients to have their diabetes well controlled prior to undergoing any cosmetic surgery.

I think your doctors are recommending 500 cc implants to make up not only for the stretched skin, but also for the thinning of the breast tissue that occurs after 20 years of having breast implants. I do not have the benefit of your photographs, but I have an idea of the problems that will be corrected, especially if there is a significant capsular contracture. There are several options for placing an implant. I usally offer an incision either around the bottom of the areola or under the the bottom of the breast. Some patients will do better with on over the other. If there is a slight droop to the breast and a lift is a consideration, then I recommend placing the incision on the bottom of the areola.
