What Would Cause my Breasts to Shrink?
Factors affecting breast volume
Jan 22, 2019
Breast volume can fluctuate with weight or hormonal changes. Beyond this, the shape can also be affected by the ligaments within the breast and the surrounding skin envelope. This can cause the volume to be redistributed so that the volume isn’t where you want it to be for ideal cosmetics. To address this, the breast tissue can be lifted and volume redistributed. An implant can be used to add volume but also improve shape. Fat grafts can be used alone, or in conjunction with an implant…
Is this rippling at 3 months post-op?
Rippling or not?
Jan 22, 2019
Its difficult to tell from the photos whether her not this is rippling. Irregular breast tissue overlying the implants can have a similar appearance. This may just be normal. In terms of fixing it – it can depend on the type of implants you already have. The reality is that all breast implants will ripple, and the degree is determined by amount of tissue and its quality overlying the implant and the cohesivity of the implant chosen. In your case, if a less cohesive device was chosen a more…
Scar revision after lollipop lift + fat transfer – Is this a good decision?
Revision of a lift and scars
Dec 21, 2018
How well this works will depend on what caused the scars in the first place – infection, reaction to stitches, surgical design, etc Either option you proposed could work – its impossible to say without an in person exam. You also want to make sure that the areas where you scarred are not associated with underlying fat necrosis (hard spots) as they will not respond well to fat grating.
How long does it take the hard spots to soften after lipo?
Softening of hard spots after lipo
Dec 21, 2018
This can vary. In most cases, things start to soften in a couple weeks and improve from there. If the hard spots are related to some bruising or compromised blood flow, these areas can persist. Usually they soften up within weeks though,
Wanting to add a little fullness to my current breast augmentation, can I transfer fat fro
Adding fat to my augmentation
Nov 20, 2018
Yes. Adding fat to a breast augmentation, also called composite breast augmentation, is often an option. Fat can be harvested at the time of tummy tuck using liposuction.Youn need to address expectations and technique with your plastic surgeon in advance though so you understand what this can and can’t achieve and possible risks.
How would a reconstruction with fat grafting with SSX445-525cc on a 5’4” 130lb look on me?
Choosing implants and fat graft to replace expanders
Nov 20, 2018
I would expect the implants in the mid-400 cc range to look good. The success of the fat graft will depend on how much fat you have to give, how receptive the tissue to which you are injecting is for fat, how much is injected and how tight the skin is from the implant. Remember that if the implants are too big, the fat graft won’t work as well, and since you have had a mastectomy, even with ADM, you have less soft tissue support and therefore some risk of displacement of your i…
Over or under muscle? Very fit and active. 5’3″, muscular, 140 lbs, with a B cup breast. W
Over or under muscle
Nov 4, 2018
Typically over muscle is associated with a bit quicker recovery and less movement of the breast implant with activity. This is balanced, in some cases, by a higher rate of capsular contracture. Above the muscle, these days, is typically subfascial. Both options have pros and cons and discussion with a board certified plastic surgeon to ensure you are comfortable with the implications of either approach may be your best move.
Can you decide after tissue expansion to go with tissue exchange instead of an implant?
Tissue instead of implant after tissue expander
Nov 4, 2018
Yes absolutely. In all cases it is critical for a board-certified plastic surgeon to determine your candidacy. But in most cases, tissue can be used to replace an implant. In our practice, the DIEP flap is most commonly used, but other options like the SIEA, latissimus, PAP, TUG, VUG, DUG, DCIA or TDAP flap may also be considered. You should see a plastic surgeon experienced in several of these options to help you make an informed decision.
Will I be in pain after breast reduction surgery?
Pain after breast reduction
Nov 4, 2018
Patients are rarely in substantial pain after breast reduction. Ask your surgeon about enhanced recovery after surgery protocols as well as nerve blocks to minimize postoperative pain and narcotic use.
Would a breast revision from 110cc to 375 to 485 be too noticeable?
110 cc jimp
Oct 27, 2018
This increase will provide some noticeable increase in fullness. I don’t think that that would be too much
32A and I’ve chosen 500cc saline, can my body handle that big of implant?
500 cc too big?
Oct 27, 2018
In most cases yes your body can handle 500 cc implants. That said, as implants are increased in size, complication rates do increase a little in terms of implant malposition. In fact the American Society of Plastic Surgeons has a separate consent form for implants over 400cc. You may consider using a simulator to see what that size may look like on you. Also if you are more active smaller implants may be more easier to manage day in and day out.
Will my plastic surgeon’s plan for revision fix my bottom out issues?
Correcting bottoming out
Oct 19, 2018
Tuberous breast’s and bottoming out are potentially challenging and without in person exam it’s impossible to provide a really precise answer. Capsulorrhaphy is a reasonable approach – it depends on how robust the capsule is. Sometimes this can be supplemented with a mesh product for support but not necessarily. Adding a larger implant can help with volume but this needs to be balanced against additional weight especially given you have already bottomed our once – but again eve…
In your professional opinion, what exactly is 360 lipo? What areas does it cover?
What is lipo 360
Oct 19, 2018
Lipo 360 is a marketing term and not a defined medical procedure. You can expect to get liposuction. Typically the 360 means it’s front and back – or all the way around – which is where the “360” comes from. But every patient has different anatomy and goals.
What causes a BA to look like the “snoopy sagging BA”?
Snoopy breast
Oct 11, 2018
To correct this the surgeon needs to adjust the relationship between the breast implant position and the overlying skin, The implant can be lowered in some cases to help. But, if the soft tissues are right, lowering more would look unnatural, or if that has already been tried and was not enough, then a breast lift and possibly adjustment of the size of the implant can be considered. The goal is to center the nipple on the implant, and for the implant to be in an anatomically reasonable location.

ACCURACY OF THE VECTRA, SLOPE AND ROUND VS ANATOMIC IMPLANTS
Oct 11, 2018
Thanks for your question. First – how accurate is the Vectra camera simulations? We have studied and published an article on this. We (and another study also confirms this) found the Vectra to be 91% accurate in terms of analyzing volume. In terms of shape and contour, there is no real study that clearly assesses this. I have simulated patients with Vectra for years and the difference between shaped and round in my opinion are really subtle. The second question relates to shaped vs…
