Routinely, the implant exchange is often less painful than the first procedure, BUT, with the increased size of your implant decision, the pocket often will require enlargement and with this there is the possibility of associated pain. I often place a drain in each pocket when doing reoperative breast surgery. Just depends on your situation. Just choose wisely a Member of the ABPS or ASAPS to be your surgeon, and you’ll do just fine.

I think your results are wonderful. Kudos to you and your surgeon. It’s been my experience over the last 2 years with Sientra implants, that your breasts will soften and drop a little more, yielding you a sillohuette that you will enjoy for many years. Since the implants are Gen 5 shaped, I think you will be pleased how natural and yet beautiful superior quadrant fullness that you will have for a long Time.

All Patients have some degree of breast and cleavage asymmetry and chest wall asymmetry. This is an intertwined anatomy that all Plastic Surgeons identify either preop or intraoperatively. I am not sure the subpectoral position would change anything. You and your surgeon have accomplished a good natural result. The overall appearance could only be changed by reducing the amount of skin present, however, this would result in scars on your breast. Often times, when the subpectoral position is chosen, skin reduction is mandatory (mastopexy). So overall you and your surgeon made a good decision.

I sympathize with your Frustrations because these Abdominoplasty scars Frustrate me as well! As a Board Certified surgeon and years in practice, the abdominoplasty scars seem to be one of the least controllable incisions that we use. It seems that the scar seems to ride higher and can widen as well. I have had to certainly lower the lateral incision due to jeans being more lower cut than in past.I have to revise some of these scars at my office under local anesthesia too….

Sorry that you have developed this somewhat rare post op problem, but there is NO upside to delaying surgical treatment if indeed this is the diagnosis. The potential problems of capsular contracture and possible infection, need to remedied with quick return to surgery for evacuation and irrigation of the pocket. This will greatly reduce the potential problems. Consultation with your PS is needed.
