Is this side swelling or dog ears? Is this normal for 3 months post TT or do I need a revision? (photo
Posterior tissue
Oct 12, 2017
Thanks for your message. Its difficult to say what is going on posteriorly because of the waistband of your underwear -- its forcing up posterior tissue. Dogears typically are excess tissue right at the edge of the incision, which is not what you have based on the pictures. However, you do have excess posterior tissue. While a revision may not be necessary, you may want to pursue additional surgery to address that, which is typically through a combination of liposuction and resection. The...
I’m 24 and have tiny boobs; Are my breasts tubular?
Tuberous breast deformity
Oct 10, 2017
Thank you for your question. From the pictures, I do not think you have a tuberous breast deformity, which is characterized by herniation of the breast parenchyma through the nipple areolar complex and by a tight, constricted lower pole and IMF. Surgery for this usually involves radial scoring of the parenchyma and adjustments to the IMF and is difficult to achieve precise and reproducible results. Fortunately, you have normal, beautiful breasts, and you should have fantastic results with a...
Why did the fat transfer to my butt turn out flat/ square from side angle?
BBL
Oct 10, 2017
Thanks for your question. At just 10 days out, you're pretty early in your post-operative period to make any long term comments on your final result. Sitting for short periods of time certainly is harmless after fat grafting, and certainly the fat grafts will shift somewhat with normal activity. At this stage, the best option is to keep in close contact and follow-up with your surgeon.

Best of luck

Lipo or Tummy Tuck?
Liposuction vs Tummy Tuck
Oct 3, 2017
Thanks for your question. From the pictures it's difficult to definitively say which operation would serve you better. Patients who are poor candidates for liposuction are those who have stretch marks or inelastic skin, typically from multiple pregnancies, since liposuction relies on the skin to contract. Also, patients with diastasis, or spreading apart of their abdominal muscles typically from pregnancy, also are better abdominoplasty candidates because the rectus plication can bring those...
Why do dog ears occur after tummy tucks?
Dogears
Oct 3, 2017
Thanks for your question. While dogears are not common, they can sometimes occur after a tummy tuck (or in any other body contouring operation). The easiest way to think of it is simply in terms of tailoring. Two areas of different lengths - the bottom of the incision and the top of the incision - are being brought to together. Because the tissue from above is longer than the tissue below (because it is more curved), that difference in length needs to be negotiated. Typically this can be...
Why do I look like this? Can you please help me?
Internal Medicine Consult
Sep 28, 2017
Given your co-morbidities, I'm unsure if you'd be a candidate of elective plastic surgery. However, you should ensure you are tied in with an internal medicine physician as well as possibly a hepatologist and endocrinologist. These are significant medical problems and you should have a clear treatment plan in place.
Good luck.
Im 5’1, weigh 117 pounds but I don’t know If I am too skinny for a BBL?
Great Candidate
Sep 28, 2017
Thank you for your question. You are a good candidate for surgery. Every patient, of course, must have realistic expectations of outcomes, and you should have a frank discussion with your board-certified surgeon regarding BBL and possible revisionary procedures. However, aggressive liposuction of your abdomen, flanks, inner and outer thighs should yield reasonable amounts of fat to process and inject. Be prepared for some downtime after surgery, and of course, review risks of surgery...
Is a tummy tuck the procedure I should go with?
Abdominoplasty with liposuction
Sep 28, 2017
Thank you so much for your question and sharing your pictures! Looking at your pictures, I believe an abdominoplasty with liposuction of your hips would provide you with your best results. The abdominoplasty would absolutely address the excess skin as well as muscle laxity you have, and the hip liposuction would help better define your waistline. Be sure to pursue consultation with a board-certified plastic surgeon, and be sure to have a complete blood count performed prior to surgery....
What to do when muscle repair fails and comes undone?
Plication Failure
Sep 28, 2017
Thank you for your question. It does appear that a portion of your plication has failed. With regards to other options, you should have a detailed discussion with your plastic surgeon. A possibility is to revisit surgery once you are several months out. This would likely be closer to six months out of surgery. At that time, another attempt at plication could be performed, or you could have any of a number of forms of mesh added to reinforce the plication. Unfortunately, as has been pointed...
Prophylaxis, breast augmentation and dermal piercings.
Prophylaxis
Sep 26, 2017
Thanks for your question. If you go by evidence-based medicine, which is to look at the data and practice accordingly, you do not need any antibiotics because of your piercings. This would be considered a clean procedure, and accordingly, you have a very low risk of bacteria going into your bloodstream or of wound infection itself.
Good luck!

5 weeks post BA surgery. Will they drop into place?
Give it Time
Sep 25, 2017
Thanks for your question. Don't worry! Your implants look great and you appear to have an excellent immediate post-operative result. Particularly with less elastic skin and with submuscular implants, I would expect your implants to feel like they are high for several months--you likely need another 4-6 weeks. They will "drop" into position as your tissues stretch and relax. Give it time; you look great.

Good luck,
Parag Butala, MD

Do I have a breast deformity? I am a 36B, 34 yrs old, no children.
Augmentation/Mastopexy
Sep 25, 2017
Thank you for your question! You have no deformity, but you do have normal, natural breast ptosis. This is very common in women. Additionally, your left breast from the pictures appears either larger than the right, or at least slightly asymmetric with a longer sternal notch to nipple distance. Seek consultation with a board-certified plastic surgeon - but be aware that an augmentation mastopexy is associated with scars that are different from typical primary breast augmentations....
Curious about the best way to handle my back rolls.
Body Lift
Sep 25, 2017
Thank you for your question and congratulations on your prior successful surgery. I agree with your plastic surgeon that an upper body lift, which would hide the scars in your bra line, would help addressing these rolls. Issues to consider prior to surgery include the addition of liposuction of the bra line and flanks at the same time, which I'd recommend, or waiting for some additional weight loss prior to further surgery. Your best results would come after additional weight loss,...
I'm 42, have the face of a 30 year old, and a neck of a 90 year old!
Necklift
Sep 25, 2017
Thanks for your question! You have excess skin in your neck. While you can attempt non-surgical facial rejuvenation, I'm skeptical you'll see dramatic results with which you'd be happy with these given the laxity of your neck skin. I would encourage you to be evaluated by a board-certified plastic surgeon; my recommendations would include a necklift that also includes the lower portion of your face. Given the proximity of your jowls, it would be relatively straightforward to correct the...
Rash at 12 weeks after breast augmentation. Skin irritation or something else?
Rash
Sep 25, 2017
Thanks for your question. Likely this will be contact dermatitis related to the clothing. You can schedule an appointment with a dermatologist, however, prior to that, try over the counter topical steroids. Should do the trick. You'll need to try other bra manufacturers too though if it's the new clothing. If it returns, you'll likely need to see a dermatologist. I doubt your plastic surgeon will have any issues with it since you're several months out of surgery already.
Best of luck!
