The only dose of antibiotics to have been shown effective is the one given prior to starting the surgery. All antibiotics carry risks even as a single, some of which can be life threatening. Most patients, 75%, in my practice are happy with a breast enlargement alone. I would do a breast enlargement first. Allow it to heal for six months and see if you like the result. Healing after surgery does take time unfortunately. Your discomfort is consistent with normal healing for some patients. It is also dependent on your sensitivity to pain. It is normal for some patients to be more sensitive to pain. Keep taking your prescribed or recommended pain medications. With time this should pass. However that can take weeks sadly. I am not a big fan of the scars breast lifts leave on patients breasts. By the look of your photos, I would recommend a breast enlargement alone. 80 to 90% of my patients are happy with the enlargement. If you have real evidence of a ruptured silicone implant, positive MRI results or changes in physical exam, your priority should not be exercise but implant replacement. Failed silicone gel filled implants exposes the body to the silicone gel which causes chronic inflammation around the implant. The longer this goes on, the greater the difficulty there will be in replacing the implant. That is why the FDA and the manufactures recommend MRI's after silicone breast implants....
