The ideal position for a buttock implant is within the muscle.  The gluteal maximus muscle is a thick muscle and positioning the implant intramuscular is ideal.  Some surgeons place the implant subfascial but the fascia is not thick enough for the implant not to show.  Underneath the muscle can lead to some possible nerve damage.  Over the muscle would show too much also.Hope this was helpful.  And thank you for your question.
