I agree that you need to see your original surgeon to discuss where you are, what you expected and what he/she can do you help you meet your goals. I also recommend seeing at least one other surgeon in your town to see how the answers compare, realizing that your original surgeon, hopefully, wants to all he/she can to improve your outcome. Size is always a personal decision. However, with that said, and looking at the pictures you posted, I think a 350 cc get you close to were you think you want to be, more like a C cup not a B. With your ht, wt, and dimensions in my opinion you need more volume then 286 cc.

Relax, your way early to be worrying about size and shape. The swelling is gone in 4-6 weeks and shape should be stabilizing by that time too. Complete healing takes 6 months but you can go bra shopping any time after 6 weeks if all goes well.

A 25 cc difference side to side is undetectable by th trained eye.  The patient may "feel" the difference but an observer couldn't tell.  As far as "loss under the muscle", nothing is lost it is displaced, and again this is a trivia difference that is impossible to quantify. Either implant will work if that is the range your choosing.

A physical examination is always the only way to give a true consult, but from the pictures and the information given you I would first need a capsulectomy of the old constricted breast to remove scar, to allow it to behave naturally, followed by a vertical or standard lift procedure. Brachioplasty can leave a noticeable scar even in the most skilled of hands. Especially living in the south were short sleeves and tank tops are common for a large part of the year. Skin tightening always lags the weight loss. I ask my folks to wait at least 6-9 after the weight loss to giveth skin time to tighten on it's own.
