You should be very proud of your weight-loss. Keep up the good work. In regards to the tummy talk you should reach your target weight before doing the surgery. Extra fat and skin on one side can usually be dealt with even without liposuction. You will definitely need a lollipop scar for your breast lift. Even with an implant you will need this in order to obtain an effective long-lasting lift. Good luck

You are correct in your assessment, as are the answers of the other surgeons. Your implant has bottomed out slightly. This has caused the scar to ride up on the breast. This is due to the inframammary fold fibers having stretched out. It could continue to get worse but no always. The most effective surgery to fix this would be to reconstruct the lower fold of the breast with acellular dermis (eg. Strattice or Alloderm) or Seri Silk. Good luck

The primary determinant of the implant size you can use is the width of your breast. In general the implant should not be wider than your breast. In order to be a double D cup size you’re probably going to need a high-profile implant in the 450 cc range. Based on your height and chest with a medium profile implant will be too wide for you. It appears from your photos that your muscles have stretched out after surgery. This is termed a rectus diastasis. It can be redone by going back in through your incision. Why this occurred is not clear. It’s possible the suture used tore or unraveled. It can also occur after weight gain or pregnancy. Good Luck.

You ask a great question. I would tend to agree that an implant under the muscle looks slightly smaller than one on top of the muscle. It also looks more “natural” or rather, less augmented. As for the size, it is best to look at photos of women who have implants under the muscle in order to compare yourself (before surgery) to their preop photos. I think you are correct to be concerned when one surgeon says it is simple and the other says it is complicated. I agree that it is a complicated surgery. The surgery has two parts. I don’t think that that grafting is the right answer for you. I think that more of your muscle has to be released. There are still fibers of your pectoralis muscle that are preventing your saline implant from sitting in the proper position. If the pocket in which the implant sits is not at least the size of the implant diameterthen part of the implant will get pushed and distorted.

You are in great shape. You have no excess skin or fat but have a lot of laxity to your rectus muscles. You also have a bellybutton hernia. I would recommend fixing this through a very low incision hidden in your bikini line. The hernia could be fixed at the same time and could even be covered by insurance.

It is way too early to tell if there is an issue. Your surgeon picked two different sized implants because he thought your breasts were different sizes before the surgery. I would just be patient and give it time. I would wait at least 12 weeks before judging any size discrepancies.
