Dear Katie,
Salicyclic acid peels and isotretinoin as suggested by Dr. Ram would likely be helpful. Less aggressive than isotretinoin are photodynamic therapy sessions (PDT). These are nearly as effective as Accutane after enough treatments, but are milder than Accutane. Surgical drainage (I do it in the office with a 20 gauge needle and an extractor) is covered by insurance and is called “acne surgery”. You will need this more than once, according to how your…

Hi Muhammad,
Thanks for your question and pictures. You have severe cystic acne affecting your face and your neck. You basically have 2 options. The first choice is Accutane (isotretinoin) which is a somewhat toxic, but very effective, therapy for cystic acne. Dermatologists can prescribe this. Your second choice would be needle drainage of the cysts following by PDT (photodynamic therapy)…

Dear hi200,
For severe acne PDT (photodynamic therapy) is almost always something I place in the treatment mix. Levulinic acid is placed on the treatment area and then activated with light after an incubation period (typically 1 hour or more). Red or blue light is typically used to activate the chemical, which then damages the oil glands in the treated area. For mild acne some patients get a 25% plus permanent improvement with 1 treatment. It is…

Thanks for your question. There are no pictures and I don’t even know your gender. If you are female consider evaluation and treatment, if present, for PCOS–polycystic ovary syndrome. There is a 3 point scale to see if you have it. First, irregular periods. Second, signs of excess androgen production such as ANY OR ALL of the following 3 things: 1) Cystic Acne. 2) Excess Hair growth. 3) Lab tests documenting high androgen levels…

Dear Keri,
For the hormonal issues you can get evaluated for possible polycystic ovary syndrome (PCOS). Most such gals have irregular periods when not on birth control pills, acne of at least moderate severity and/or cysts on an ultrasound of the ovaries. Spironolactone pills are often helpful for acne whether or not you have PCOS. You want permanent results. See a local skin expert who performs PDT (photodynamic therapy) and you will have a permanent…

Dr. Mr. Patel
Thanks for your question. I assume, perhaps correctly, that living in Australia and having a name like Patel you are of Indian origin. Most Indian skin is fairly dark. Many lasers, including CO2 lasers, are problematic in dark skin, often leading to hyperpigmentation. Recommending a laser without knowing your skin type is problematic. The following assumes you have Indian skin of the typical dark color, such as VJ Singh, the golfer. If your…
