I'm going to assume you had a flap done from the abdomen which apparently failed. Unfortunately, now that you’ve had an infection with the implant; your instance of contracture or tightening is almost 100%. Unfortunately the only option that will work for you at this point is a flap. By using autologous tissue and removing the implant you should be able to get a normal result.

Since the abdominal tissues have most likely already been used, you would need a tissue such as PAP flap or...

Once an infection sets in after implant reconstruction, it becomes very difficult to reconstruct that breast with an implant in the future. Instead you should be switched to autologous tissue.
