What treatment options do I have to remove excessive fat from posterior lower face and apples of
Facial fat deposits
Aug 17, 2022
Hello and thank you for your excellent question. Fat in the face proper is a bit challenging as one should not perform liposuction in this area (above the jawline) so as not to risk contour irregularities. An option you may consider and most likely would be my recommendation is a facelift approach for direct defatting of this area with the benefit of suspending the soft tissue as well – an advantage for the common issue of weight loss laxity of this area. Consult with a plastic surgeon…
What type of incisions can I anticipate with a neck and facelift procedure?
face and neck lift incision location
Jul 28, 2022
Hello,

Thank you for your question. From the photograph submitted, it seems like you would be best served with a face and neck lift to address the laxity and descent of the mid-face, lower face, and neck. The neck bands are a feature of the platysma muscle which is intimate with the overlying skin, producing the central neck bands. To correct these issues, an incision would be located from the temporal area around the front of the ear (hiding the incision at the seam of the ear), around…

Am I candidate for a breast lift without implants?
Breast ptosis with satisfactory volume
Jun 14, 2022
Good morning Heather,

Thank you for your inquiry. I agree that you have sufficient volume. The ptosis (sagging) and asymmetry (left lower than right nipple position) can both be nicely corrected with a circumvertical breast lift. I think you would be very pleased with the result. The lift would leave a ‘lollipop’ shaped scar pattern: around the areola and from the areola to the inframammary fold under the breast. The lift would make your breasts look more youthfully shaped and even. …

Breast augmentation with or without a breast lift
Mar 15, 2022
Hello,

Thank you for your question and submitted photos. From those, there appear to be two issues: lack of fullness of the upper breasts compared to your wish photos and a lower position of your areola. An augmentation would improve the fullness to the extent that you wish, but you may honestly wish to consider a breast lift – either a periareolar or vertical lift depending on the size of implant desired. I do think that you would be very pleased with the result and accepting of the…

How can I fix all this extra skin in my armpits?
Excess skin of axilla (armpit)
Jan 25, 2022
Hello,

Thank you for your question. This is not an uncommon area to desire improvement after weight loss. As in other areas of the body (breasts, tummy, thighs, arms) after massive weight loss, when there is a large amount of excess/redundant skin, the best option is direct excision/removal of the skin in as inconspicuous a manner as possible – leaving the resultant scars hidden. For these areas, an excision of the excess skin may be done with placement of the scar in the skin crease of…

Should I go for liposuction or CoolSculpting for hips and thighs?
Liposuction or Coolsculpting
Mar 15, 2021
Good morning,

Thank you for your great question. From your images and the scope of the localized fat deposits to reduce, I recommend liposuction over Coolsculpting. It has the advantages of being one procedure and I believe it would be easier to do a thorough reduction of these areas. Coolsculpting typically takes multiple procedures and I have not seen results in patients such as yourself that are as good as tumescent liposuction. Consult a plastic surgeon certified by the American…

Should I consider a lip lift to correct my uneven lips?
Lip lift for asymmetry
Feb 23, 2021
Good morning Kaylee,

Thank you for your question. It is difficult given the photograph to accurately evaluate the asymmetry and the vertical length of your upper lip – which may be distorted by the lens. The three issues to define are the slight lack of fullness of the left upper lip compared to the right, the true vertical height of the upper white lip as a whole, and the slight increased vertical height of the left upper white lip, again compared to the right. As you know, an upper lip…

discoloration after filler
Feb 22, 2021
Hello,

You don’t state how long it has been from filler placement, but this does not appear to be a bruise and it would be thought to be vascular compromise to the skin. You should be examined promptly and intervention recommended.

Best,

Brad Remington, MD

Do you have to wait until filler is completely worn off before re-treatment?
Filler repeat treatments
Feb 22, 2021
Good morning Monica,

Filler may be retreated after a couple weeks. Oftentimes, it is better to do an initial volume fill and see the effect before perhaps overtreating an area. As filler is priced by the syringe, there unfortunately is no discount, however I usually do discount a second syringe if the time after the first treatment is short for the above reason.

Cheers,
Brad Remington, MD

Will I have a weird gap because my breasts are so far apart?
separation of breasts after augmentation
Nov 20, 2020
Hello,

Thank you for a commonly asked question. Actually, from your photograph, your breasts don’t appear to be more separated in the midline than the average patient. Since we must accept the position of your breasts on your chest and be mindful that placing implants more toward the midline can result in 1) more visibility and palpability (feeling of the implant) and 2) possibly displacing the implants laterally (toward your upper arm), it is unwise to create the implant pockets too…

Would fillers even out my single dimple?
Facial asymmetry
Oct 29, 2020
Good morning and thank you for your question. It would be difficult to surgically create a dimple for symmetry with the risks outweighing the benefit. I do think that a filler would be the best option and the nice thing is that you could try a shorter lasting filler to see if that is what you want as the filler would absorb in several months. If that produces an excellent result, then a longer lasting filler could be used.

I have fillers in my face and I need to get an MRI of my head. Will this affect the fillers?
fillers and MRI scan
Oct 18, 2020
Thank you for your question. I don’t know of any contraindication of having an MRI scan with commonly used facial fillers. Usually metallic devices are the concern.

Cheers,
Brad Remington, MD

What is the safest/best route for natural looking breast enhancement and nipple reduction? (Photo
breast augmentation and droopy nipples
Oct 15, 2020
Thank you for your excellent question which applies to many women. It appears from the photographs submitted that you would benefit from a nipple shortening procedure. A technique that I favor is removing a “cuff” of skin at the base of the nipple and bring the tip of the nipple back to the base. This preserves the nerve supply to the tip of the nipple to keep the very important sensation. For the augmentation, I would recommend an inframammary fold incisional approach (the fold under…
Botox for crow’s feet on cheeks? Should I try filler instead?
Botox for lower crow’s feet wrinkles
Sep 23, 2020
Hello and thank you for your good question. Given your photo, it would appear that Botox is the best treatment option to soften the lower wrinkles. The goal is to weaken the muscle causing the wrinkle rather than fill under the muscle with filler. The Botox injection can be ‘fanned out’ along the orbital rim and will likely produce the result that you desire.

Excess filler of lips
Aug 28, 2020
Hello and thank you for your question. Typically, it is prudent to wait for at least several days or longer to decide if excess filler has been injected. This is because usually it will improve as the swelling from the injection procedure resolves. Icing as you’ve been wisely doing is helpful during this time. If after this time, you remain dissatisfied, hyaluronidase injection may be used to make the filler absorb sooner. Massaging the lips may also be helpful. It would be helpful if…
