Based your photos you are a very good candidate for a liposuction abdominoplasty. First, I would address the extra fat with liposuction of the entire abdomen especially the middle part and the muffin top. The muscles have almost certainly separated from childbirth. They would be repaired. Then the skin would be brought down and contoured to remove the extra skin keeping the scar very low. The belly button would be repositioned.

Total capsulectomies on both sides with implant removal without replacement is a good idea. You will also need a complete mastopexy on both sides for the best shape. The previous peri-areola mastopexy will probably have no effect on blood supply to the nipples, but it would be a good idea to review the operative report from your previous surgeries to be sure. However no particular reason has been supplied that would require the removal to be en bloc. Your implants can be replaced with new ones at the same time, if you wish. When saline implants start to leak they feel emptier and softer. I think that is what you mean when you say the consultant can feel a rupture. Saline implants can deflate and the free saline is absorbed. So assessing the size comparison with the other side or prior photos is important. If you and your plastic surgeon agree that the shape is changing and the size is getting smaller, then it is time to replace the implants. An MRI is not necessary.

If one breast is becoming form and moving up, then you might have capsular contracture. You seem to have a small frame and rather large implants. Larger implants are heavier and often become unsatisfactory in quite a short time. I always caution patients who want very large implants that larger the implant shorten the time to the next revision surgery.

I always prescribe antibiotics with breast implant surgery, but there is no good proof that this is effective. Absolute meticulous sterile technique is very important. The incision under the breast is better than the areola incision to avoid contamination of the implant from dormant bacteria within the breast ducts. The Keller funnel might help. Exparel is a long action numbing medication and helps with pain control. Prineo is a dressing that is applied in surgery and you do not need to do anything to the incisions afterwards.
