I think this is a personal option of the surgeon.  I use a long lasting dissolving suture however may will use a permanent suture

The risk of blindness is related to bleeding that extends into the posterior part of the eye and puts pressure on the optic nerve.  This is a very rare event but it has been reported and can happen

You seem to be developing ptosis of the upper eye lids.  There may be medical reasons for this and you should be evaluated by an Ophthalmologist

It looks like you needed a breast lift along with the implant placement.  That would allow for correction of the asymmetry.  The down side is that it Will place added scars on the breast.  All is not lost as the lift can be done as a second stage.

This a long time and an infected implant needs to be considered.  Have there been any cultures and have you been on antibiotics?  i would suggest a second opinion.

For the most part it is better to have the reconstruction after the radiation.  Also we do not to do an implant reconstruction in the face of radiation.  There are a number of cases where a tissue expander is placed at the time of the mastectomy and kept there as a spacer.  The radiation is given with the expander in place and then removed and the breast reconstructed with a perforator flap.  This has been very successful in our patients.

The complication rate may be higher at this weight however if you are medically stable the procedure can be done safely.

Since i do both procedures there are a number of technical issues that go into my decision.  I would suggest that you go over with your doctor why he feels one technique is better for you than another technique.
