Statement: Daniel J. Pyo, MD, FACS, FAAP, FACCWS, is a board-certified plastic surgeon and specializes in cosmetic and reconstructive surgery. Additionally, Dr. Pyo specializes in body contouring procedures, abdominal wall reconstruction, hand surgery, wound care, pediatric cleft lip and cleft palate surgery. You are on the right track. A mastopexy is the general term for a breast lift. There are several techniques that will get you there depending on the degree of ptosis, or droop that you have. It also depends on the comfort level of the surgeons, as not all surgeons perform every type of lift. I agree that you probably had some of the sutures that were used for the plication of the fascia either break or loosen. That has allowed the repair in that location to become disrupted. I don’t think this is due to a hernia. Unfortunately, without repairing this situation, you will have worsening of the bulge, as more stress will be applied to the repaired area just above the bulge. You are correct in your understanding that Botox causes muscle weakening. Prolonged paralysis of the muscles will cause progressive weakening of those muscles. The weaker they get, the longer they will take to recover their strength. That leads to a prolonged period where the creases will not be as deep. It is difficult to judge the degree of crookedness of the nose without looking at some pre-operative photos. A lateral view is helpful for looking at the amount of columellar show that is present. The nose looks appropriately swollen, and that may be contributing to the appearance of the tip and columella. The base of the columella does look wide, but again too early to tell for sure. Would definitely wait at least 5-6 months before making any definitive decisions…

Yes, Exparel is effective for 72 hours and has been used in many different procedures, allowing a decreased need for narcotic analgesics. It is comprised of a long acting local anesthetic, Bupivacaine, in a liposomal bubble that is released slowly. I have been quite pleased with the results so far, and will continue to recommend it to my patients in the future.

It does look like the R. breast has a hematoma that should be addressed as soon as possible. The presence of blood in the pocket will predispose you to developing a capsular contracture as well.

The redness that appears in the photos along the incision line looks like a contact dermatitis, likely from the steri-strips. It does not look like an infection at this time. Good luck.

Lucy, this hematoma is fairly mature by now, and will have a fairly thick and established capsule. You should see if the ultrasound was able to determine how thick the capsule is. If it is a fairly thin capsule, then aspiration alone may do the trick. If it is a thick capsule, then if may require removal through an incision. Liposuction will not completely address the hematoma either. I would definitely advise dealing with the hematoma prior to the liposuction…

As the other responses have indicated, this appears to be slightly more than typical bruising after this procedure. The bruising stems more from the liposuction than the tummy tuck and should resolve over a prolonged period of time. These rarely require operative intervention for drainage. So the long and short of it is that it is within the range of normal, and should resolve without any management.

The issue that you raise occurs when the muscles still have enough activity to contract. This arose because the amount of Botox injected was not enough for the muscle mass you have. To address this problem, I would administer more Botox to paralyze the muscles that are still active.
