It is impossible to know after this very limited contact what might be causing your pain. A Tummy Tuck can probably improve your APPEARANCE (it is difficult to say for sure from the photo you posted), but should not be expected to improve the pain you are describing- and in fact might make this worse.
There are multiple things that might be causing your pain, including a neuroma (when a sensory nerve becomes involved in scar tissue), scar contraction (tightening of your scars causing pain), or abdominal wall syndrome. The best way to proceed would be to find a reputable general surgeon who can work in conjunction with your Board Certified Plastic Surgeon to accomplish your goals and give you the comfortable and aesthetic result you want. Tummy tuck after multiple surgeries on the abdomen
You could have a great looking tummy after a tummy tuck with most of the old scars removed and a nice, flat result. However, you need to lose weight first because from your photo you are not presently a good candidate. Also, the pain you experience, if it is from adhesions, will still likely be there becausde the tummy tuck doesn’t go inside your abdomen to address anything inside. (Richard P. A tummy tuck (abdominoplasty) gives the best results in non-obese individuals who are at a stable weight and who do not plan on having more children. The operation will remove the skin and subcutaneous fat from the belly button to the pubis, flatten the tummy by putting the rectus (six pack) muscles together, narrow the waist and lift a sagging pubis. But – the operation will do nothing for recurrent intraabdominal pain from presumed adhesions. If symptomatic (recurrent bouts of nausea and vomiting etc) they may need to be released surgically. Tummy tuck can remove skin scars or stretch marks of the lower belly wall and reshape the belly button. It can also address stretching of the muscles and fascia of the abdominal wall. In your case, it can certainly change the appearance of your tummy.

An abodominoplasty or tummy tuck will help the external appearance of your abdomen and remove any scars that are between your belly button and your pubis. However, these procedures will not improve any pain related to adhesions in your abdomen since the tummy tuck just involves the external layers of your skin and fat.

When you have scars on your abdomen you have to be extra careful with a tummy tuck. As all of your scars are below the belly button then it is more likely that they will be removed and not get in the way of the procedure.
