Tummy tuck with stretch marks
Based on your pictures, you would be an excellent candidate for an abdominoplasty.
You have a relatively low belly button and a long torso.

Liposuction helps to thin areas of excess fatty tissue but does nothing for loose skin and stretch marks. Based on what I’m seeing in your pictures, trying to liposuction in somebody as thin as yourself would only cause more problems. You would be a better candidate for a skin excision procedure such as a tummy tuck or mini tummy tuck. These procedures would leave a scar across the lower abdomen.
If a full tummy tuck is performed there would also scar around the inside of the belly button but this would allow us to remove more skin and distribute the tightening to the upper abdomen.
I would need to examine you in person to get a better idea how much laxity there is in your skin and abdominal muscles before making more definitive recommendations and counseling you about the most likely length and location of the scars.
In my experience, though, most women who have had 4 pregnancies have enough laxity of their abdominal muscles to benefit from tightening the underlying muscles and enough laxity in their skin to remove a significant amount and make the tummy tuck worth while. You are clearly not a candidate for liposuction as you do not have any excess fat. You do appear to have alot of excess skin and are probably a good candiate for an abdominoplasty which should give you a “ripped” abdomen thugh obviously an examination is needed. (Frederic H. I do not think that liposuction would be a good choice for you. You have very little fat and loose skin with stretch marks in this area. You would be better served with a Tummy tuck which would remove a good portion of the stretch marks, reshape the belly button, and tighten the skin in the area. Due to the fact that your are very fit, there is little to no fat in your abdomen and you would not achieve the pre- baby look on your abdomen. It is best to consult with another surgeon who is offering a package of options. Possibly a mini tuck, with Venus freeze and derma pen will get you where you want to go. Again, I implore you to see several surgeons and read on all aesthetic options as well that will achieve the result you are looking for.
Surgery may not even be needed. Find a board certified plastic surgeon in your area who has a before and after gallery of photos of similar patients clientele that have achieved results you are looking for. The best thing to do is see still another surgeon. There is significant excess skin above the belly button and NO excess fat. Liposuction will undoubtedly cause skin irregularities that can’t be fixed. You are a good candidate for a full tummy tuck, probably without muscle tightening. Your anatomy is on commonly seen. You have a number of options. A simple skin excision from your lower abdomen (scar will be approximately along the bikini line like a long version of a C section scar) will tighten only the skin below your belly button, and it will distort your belly button exactly as it does when you pull down the skin as shown.
When you sit down or bend forward the skin above your belly button will be exactly the same as it is now. If you have some form of mini tummy tuck (surgeons tend to define mini tummy tuck a number of different ways) that involves what we call “translocation of the umbilicus” you will end up with tighter skin both above and below the bb, but you will have a scar around the bb in addition to the lower bikini line scar after tummy tuck.
I would venture to say that you personally will also have a small vertical scar somewhere between your new bb and your lover horizontal scar because the bb defect will probably not come out. It doesn’t look like you need muscle tightening but this is impossible to tell without examining your abdomen. Bottom line is that you have options but will have to decide if they are worth the scars that are required to achieve them. BTW. Liposuction will just make your tummy look worse!
