Yes fat transfer can be performed even if a patient has breast cysts. You can even develop more cysts after fat grafting that are little bits of fat that did not survive called oil cysts. These kinds of cysts are not a problem or anything to worry about.

I recommend to all my patients to have annual screenings. A physical exam is all that is needed for a capsular contracture. If you are just checking the integrity of the implant I tell my patients to get a breast MRI without contrast. If you are concerned with the breast tissue get a breast MRI with contrast.

Well certainly the risks are different. With fat transfer you can develop oil cysts or fat necrosis, but can easily be seen on breast screenings. Fat transfer is good for patients who are only looking to increase by a cup size and have enough available fat. With Breast implants you risk implant rupture or capsule contracture, but if you are looking to increase your size by more than just a cup size implants can be a better option.

Here in the U.S. we have a smaller percentage of patients that have textured implants. A large reason for that is recently the FDA issued a recall on all Allergan textured implants due to recent findings with ALCL (Anaplastic large-cell Lymphoma). Currently I am recommending smooth implants until more is known about ALCL.

If you are wanting implants, I would suggest getting an implant that has the cohesive gel. These type of gel implants are much stronger and can withstand a lot of force. However any direct force could still rupture the implant. I always recommend my patients to start getting breast MRI screenings after the third year they have had the implants and every other year after that to make sure that the implants are still intact. With these newer implants they get what's called "silent rupture"...

As long as your Hashimoto's is under control and you are being followed by your primary care physician or endocrinologist you should be able to get breast implants. However, your surgeon might require clearance from your managing physician.

In my experience silicone implants due better when placed subglanular. The reason for this is that silicone implants will always have less wrinkling. Because you are not placing the implants under the muscle any wrinkling or imperfections will more visible.

I believe that fat transfer gives a nice result for patients that are not wanting breast implants or for patients that would not be a good candidate for implants. Fat transfer breast augmentation is meant for patients that want a small cup size. It is also always good to know that with fat transfer that little cysts can become present due to fat necrosis. In my experience these are very common and almost everyone who has this procedure gets them. They normally feel like little lumps. Some are visible and others are not. You have to think of it this way, you are taking these cells from one area that has established circulation and put it into another area that now needs to reestablish circulation. Personally, I love the GalaSHAPE product. It provides support where needed especially for women who have larger breasts. I use it in my breast reductions, breast lift and even my breast reconstruction cases. This is an excellent question! Big events such as weddings are always a popular time for surgery. I tell my patients that I would ultimately like them to have 6 months before the big event. That way all of your swelling should have subsided. However if you are a little short on time I do say you can do 3 months before the big event but know that you could possibly have some residual swelling.

Generally I recommend my patients wait at least 6 months. This way it gives the time for the swelling to go down and subside. It also gives me a good idea of what you have left giving me a good baseline. A good baseline is the foundation of what makes fat transfer successful.

This is a concern for a lot of my patients. I think that it is important that you find a plastic surgeon that specializes in beast augmentation. In my experience most women have not lost nipple sensation after a breast augmentation. However it is a risk, it is pretty rare in my opinion.
